Community Health Outreach is a collaborative effort between The Salvation Army and the Onondaga County Health Department. Community Health Outreach is designed to improve access and utilization of primary and preventive health care services for pregnant and parenting individuals while also providing assistance in the areas of housing and income.  Participants may be older than 21 years of age if they are currently caring for children less than two years old.  Participants and their children benefit from ongoing home visits, advocacy and community referrals. Community Health Outreach offers an extensive pre-natal and post-natal educational curriculum, tailored to meet a variety of individual needs.
